				<title>Allergic Diseases and Patient Safety: Bridging a Critical Gap in Clinical Care</title>

				<abstract_fa><![CDATA[]]></abstract_fa>
				<abstract><![CDATA[Allergic disorders—ranging from asthma and allergic rhinitis to atopic dermatitis, food allergy, and drug hypersensitivity—affect nearly one-third of the world’s population (1,2). Over the past decades, their prevalence has risen steadily, especially in urbanized societies. While the public health and economic implications of these conditions are well known, their significance as a patient safety issue is less frequently emphasized. In daily clinical practice, I have witnessed how allergic reactions, particularly anaphylaxis, can emerge abruptly and turn into life-threatening emergencies. Many such events are avoidable if appropriate preventive strategies are in place (3). Addressing allergies through a patient safety lens is, therefore, both a medical and ethical necessity.]]></abstract>

				<title>Impact of a pre-procedural checklist on complications in emergency endotracheal intubation: a prospective cohort study</title>

				<abstract><![CDATA[Introduction:Emergency endotracheal intubation (ETI) is high-risk. Pre-procedural checklists may enhance safety, but evidence in emergency ETI is limited. This preliminary, exploratory study evaluated a checklist&#039;s impact on ETI complications and mortality. Materials and Methods: This prospective cohort study enrolled adult Emergency Department (ED) patients requiring emergency ETI (August 2021-December 2022). A control group (pre-checklist) was compared to an intervention group (post 12-point checklist implementation and training). Data on procedure-related complications and 24-hour mortality were collected and analyzed using relative risks. Results: 131 patients (control n=68, checklist n=63) were included. The overall composite complication rate was similar (RR 1.01, p=0.90), potentially masking individual effects. Checklist use was associated with reduced aspiration (RR 0.36, p=0.045) but an unexpected increase in post-intubation hypoxia (RR 3.24, p=0.004); reasons, possibly procedural delays or altered team dynamics, require further investigation. Other individual complications and 24-hour mortality (RR 1.38, p=0.22) showed no significant differences. Conclusion: In this underpowered, preliminary, and exploratory study, a pre-procedural checklist did not significantly reduce overall ETI complications. The observed decrease in aspiration alongside increased hypoxia warrants cautious interpretation and underscores the complexity of checklist implementation in emergency airway management. Further research with larger sample sizes is crucial to optimize checklist design and confirm its impact on patient safety.]]></abstract>

				<title>Comparative evaluation of enoxaparin and rivaroxaban treatment outcomes in cancer patients with venous thromboembolism: a prospective cohort study</title>

				<abstract><![CDATA[Introduction:
The purpose of this study is to assess and compare the efficacy of Rivaroxaban and Enoxaparin in the management of Venous Thromboembolism (VTE) in cancer patients, as well as to evaluate potential risk factors associated with VTE recurrence.
Materials and Methods: 
A retrospective cohort study design was employed, analyzing 402 patients above 18 years of age who were diagnosed with cancer and treated for VTE at Imam Reza and Ghaem hospitals between 2023 and 2024. Exclusion criteria were also set, and treatment regimens were detailed for both drugs. The analysis was conducted using chi-square tests, independent t-tests, and logistic regression.
Results: 
The data revealed that Rivaroxaban was used more frequently than Enoxaparin, although the mean age was higher in the Enoxaparin group. No significant difference was observed in terms of VTE recurrence between the two drugs.
Conclusion: 
The study concludes that Rivaroxaban and Enoxaparin are both effective in treating VTE in cancer patients. The selection of medication may depend on the specific characteristics of each patient.]]></abstract>

				<abstract><![CDATA[Introduction:The increasing prevalence of antibiotic-resistant bacteria, particularly β-lactam-resistant strains, has become a serious global health concern, limiting the effectiveness of conventional antimicrobial therapies. Gastrointestinal pathogens such as Bacillus cereus, Salmonella typhimurium, and Escherichia coli are among the common organisms associated with resistant infections. In response to this challenge, medicinal plants like Terminalia chebula, known for their broad-spectrum bioactivity in traditional medicine, are being explored as promising natural sources of antimicrobial agents. This study aimed to evaluate the antibacterial activity of ethanolic and propanolic extracts of Terminalia chebula against gastrointestinal bacteria resistant to selected β-lactam antibiotics. Materials and Methods:Ethanolic and propanolic extracts were prepared by cold maceration and analyzed using gas chromatography–mass spectrometry (GC-MS) to identify major bioactive constituents. Antibacterial activity was assessed using agar well diffusion assays, and minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) values were determined.Results:GC-MS analysis revealed that 1,2,3-benzenetriol (pyrogallol) was the most abundant and the most important antimicrobial compound in the ethanolic extract. In the propanolic extract, propanoic acid was identified as the most abundant constituent; however, D-limonene was determined to be the most potent antimicrobial compound. Both extracts exhibited considerable antibacterial activity against all tested strains, with B. cereus being the most sensitive, followed by S. typhimurium and E. coli.Conclusion:The antibacterial effects observed can be attributed to key phytochemicals such as pyrogallol and propanoic acid, which may act by disrupting bacterial cell walls and interfering with metabolic functions. These findings highlight the potential of Terminalia chebula  as a natural therapeutic agent against drug-resistant infections. Further in vivo studies and clinical evaluations are warranted to explore its clinical applicability.]]></abstract>

				<abstract><![CDATA[Introduction:
The fundamental principle of &quot;first, do no harm&quot; underscores the importance of safety in healthcare, particularly in surgical settings where preventable errors may occur. The World Health Organization&#039;s concept of safe surgery emphasizes accurate surgical site marking to reduce such risks. Healthcare workers’ awareness of this practice plays a vital role in ensuring patient safety. This study evaluates the prevalence and compliance of surgical site marking in an orthopedic clinic.
Materials and Methods: 
This retrospective study reviewed patient records from the Hospital Information Management System of a university hospital&#039;s orthopedic clinic. It focused on pelvic and knee surgeries performed between January and March 2022. Out of 106 patients, 2 were excluded due to inaccessible surgical site marking forms, resulting in a final sample of 104 patients.
Results: 
The mean age of patients was 54.4 years; 55.8% were female. Among the surgeries, 65.4% were elective, and 54.8% lacked surgical site marking. A significant association was found between the type of surgery (emergency vs. elective) and the presence of surgical site marking (p&lt;0.05). Furthermore, surgical site marking status was significantly related to average hospital stay duration (p&lt;0.05).
Conclusion:
Despite its critical role in patient safety, surgical site marking was often overlooked, especially in emergency surgeries. The findings highlight the need for improved compliance in orthopedic practices. Educational interventions, particularly those supported by artificial intelligence, are recommended to increase awareness and implementation of surgical site marking protocols.]]></abstract>

				<title>Comparison of microbial culture results before and after implementation of an icu-acquired infection prevention protocol at imam khomeini hospital, ardabil, iran</title>

				<abstract><![CDATA[Introduction:Nosocomial (hospital-acquired) infections, particularly in intensive care units (ICUs), pose a significant challenge to healthcare systems. Effective control of ICU-related infections is crucial, as it can reduce patient mortality rates and shorten hospital stays. Materials and Methods: In the interventional study, 74 samples for microbial cultivation were selected of different parts and equipment and sent to the laboratory for analysis. Subsequently, the ICU departments underwent thorough cleaning, and staff received training on proper hand hygiene protocols. Post-intervention sampling was then conducted. Data from both pre- and post-intervention measurements (two sampling rounds each with 37 samples) were analyzed and sent to the laboratory. The microbial culture results were recorded in the researcher-designed checklist for further evaluation and collected data were analyzed by statistical methods in SPSS version 24. Results: Microbial contamination analysis revealed coagulase-negative Staphylococci, Escherichia coli, and Citrobacter freundii as the most prevalent pathogens (25% each) in pre-intervention samples. Following decontamination protocols and staff training, post-intervention cultures showed a reduction in overall contamination, with coagulase-negative Staphylococci and Acinetobacter spp. emerging as the dominant species (20% each). This observed decrease in microbial load suggests the educational intervention may have contributed to improved infection control practices.Conclusion: The results demonstrated a significant reduction in Gram-positive and Gram-negative bacterial contamination across both medical equipment and hospital surfaces following the decontamination protocol. This observed decrease in microbial load, particularly of pathogenic species, suggests the infection control training program effectively improved sterilization practices among healthcare staff.]]></abstract>

				<abstract><![CDATA[Introduction:
Suicide is an act intentionally carried out by an individual to harm themselves, fully aware of the consequences. Emotional and relational factors also play a significant role in suicide attempts. Psychologists associate suicide with low self-esteem and feelings of emptiness.
 Materials and Methods: 
This descriptive study, conducted from 2021 to 2023, involved sampling patients admitted to Amir al-Momenin Hospital in Zabol. A checklist was completed for each patient diagnosed with a suicide attempt, covering demographic data, methods of suicide, psychiatric history, and other relevant factors.
 Results: 
Methods of Suicide:

Males: 2% drug overdose, 11% pesticide poisoning, 13% hanging, 3% self-immolation, 5% other intentional injuries.
Females: 27% drug overdose, 30% pesticide poisoning, 2% hanging, 4% self-immolation, 3% other intentional injuries.
A significant association was found between suicide methods and gender (Fisher’s Exact Test, P-value = 0.01).

 Conclusion:
Suicide is a serious global public health issue. Patterns of suicide vary based on individual characteristics, methods, causes, and socio-cultural contexts.]]></abstract>
