				<title>Quality of Anticoagulation Management with Warfarin among Patients in Orotta National Referral Hospital, Asmara, Eritrea: A Retrospective Cross-Sectional Study</title>

				<abstract><![CDATA[Introduction:To analyze the quality of anticoagulation management with warfarin in Orotta National Referral Hospital, Asmara, Eritrea. Materials and Methods: A retrospective cross sectional study was conducted on patients aged 18 years and above who had received warfarin for Deep Vein Thrombosis and Valvular Heart Disease for at least 30 days. Data were collected by reviewing patients’ medical records. Time in Therapeutic Range (TTR) and International Normalized Ratio (INR) were used to analyze the quality and factors that affect the anticoagulation management. Results: Out of the 336 patients studied, 24.5% and 28.8% of patients, with Deep Vein Thrombosis (DVT) and Valvular Heart Diseases (VHD) respectively, had INR measurements within therapeutic range. The mean TTR was 25.1%, while the mean duration of INR monitoring for all patients was 40 days with a standard deviation of 43 days. The daily dose of warfarin was increased in 35.4% of participants following subtherapeutic INRs, and decreased in 50.6% following supratherapeutic INRs. No association was found between optimal TTR outcome and concomitantly used medications as well as comorbid conditions. However, the use of alternate dose regimen was found to be significantly associated with in-range (therapeutic) INR measurements. Conclusion: The study concluded that the quality of anticoagulation management with warfarin among patients in Orotta National Referral Hospital was suboptimal. This was demonstrated by low mean TTR, longer INR monitoring frequency and unsatisfactory measures taken to adjust warfarin dose after occurrences of non-therapeutic INRs.]]></abstract>

				<title>To Increase the Practice of Hand Washing with the Correct 7-Step Technique for 2 Minutes for Doctors Conducting Vaginal Deliveries from Baseline to 80 % in 4 Weeks</title>

				<abstract><![CDATA[Introduction:Hand washing is one of the basic habits to prevent infection from service provider to service seeker. Germs are transmissible from one person to another. Hand washing is an effective measure to prevent infection in a health care setting. Local problem: In the labour room, it was found that doctors who are conducting deliveries are not doing hand washing with the correct steps for 2 minutes as often as they should. Materials and Methods: A quality improvement team was formed. Fishbone analysis was performed to determine handwashing status, and five plan-do-study-act (PDSA) cycles were completed to implement a change intervention, with one change being tested, implemented, studied, and adapted for the next PDSA cycle in each cycle. Interventions: Hand washing resources were made available to students, awareness messages were distributed, and live demonstrations and interactive hand washing instruction were conducted in the labour room. Results: Baseline data showed that 10% of doctors were hand washing; a change intervention was implemented after the first PDSA data rose to 55.5%; further PDSAs 2, 3, and 4 were completed, and handwashing rates improved to 69.1%, 66.1%, 75%, and 80%, respectively. Conclusion:It is very important to identify system dysfunction. Acceptance of the problem is half the solution, and by applying simple methods of quality improvement by an enthusiastic team and continuing motivation to make the desired change happen here, we could change the habits of junior residents.]]></abstract>

				<abstract><![CDATA[Introduction:The complexity of home care is demanding in terms of patient/client safety. The aim of this study was to explore patient/client safety structures and processes within organizations providing home care during the reform of home care and the supervision of patient/client safety in Finland. Materials and Methods: A cross-sectional exploratory online survey for persons responsible for safety strategies at home care organizations (home nursing, hospital-at-home, home services). Results: The majority of organizations (89%) had a patient/client safety plan, which included central safety strategies and responsibilities. However, the authority responsible for the various dimensions of patient/client safety varied. Regarding responsibility for the supervision of care personnel’s surveys and reports of the hazardous and adverse events, a significant difference was seen between organizations providing both home nursing and social services versus only social services (94% vs. 49%, [CI 34%-64%] p &lt;.001). Patient and client centered measures varied from patient/client service and care consultation processes (81%) to patient/client safety observation rounds (24%). Care personnel were seen to have a function as Everyday safety “watchdogs” or “Safety whistleblowers”, and employ a broad range of preventive and reactive methods for patient/client safety. Conclusion:Most organizations were seen to have well-established, organizational patient/client safety structures and processes. The methods used to identify, prevent, minimize, and report safety problems were mostly proactive, indicating an active, analytic, and learning safety culture. Current challenges included involving service users and new employees in safety processes to create a truly comprehensive and sustainable safety culture.]]></abstract>

				<abstract><![CDATA[Introduction:The increase and diversity of diagnostic and therapeutic methods using rigid or flexible endoscopes have focused much attention on the possibility of infection transmission in this way. Therefore, nursing personnel must have basic information on sterilization and disinfection of tools.Materials and Methods:This descriptive-analytical study aimed to evaluate the staff’s performance and awareness of healthcare instructions for proper disinfection of endoscopic consumable and non-consumption equipment in the hospital affiliated with Mashhad University of Medical Sciences, which had endoscopy departments. The data collection tool was the standard endoscope decontamination evaluation checklist taken from the translation of the checklist of WHO and analyzed by SPSS 20 statistical software.Results: The results showed a statistically significant difference between nurses&#039; knowledge and performance (P-value = 0.03), and nurses&#039; performance had lower percentages than knowledge.Conclusion:Due to the statistically significant difference in nurses&#039; knowledge and practice in correct disinfection of consumable and non-consumable equipment in endoscopy and the low performance of nurses in some areas of the disinfection process, interventions such as face-to-face training of nurses and continuous monitoring of performance, especially evaluation of three months of nurses&#039; performance are necessary.]]></abstract>
