				<title>Exploring the Relationship Between Perceptions of Safety Culture and Patient Safety Events in Inpatient Clinical Teams</title>

				<abstract><![CDATA[Introduction:Inpatient clinical teams in hospitals must communicate properly to maintain a culture of safety. The purpose of this study was to understand how perceptions of patient safety culture relate to the frequency of safety events in hospitals. Predicted connections were made between elements of safety culture and safety events, using Schein’s model of organizational culture as a framework. Materials and Methods: The research team was able to gain access to a large sample of perceptions of safety culture from clinical teams in hospitals. Results: Results showed that handoffs and transitions were a significant predictor of the reduction of safety events, whereas other predictors were not significant. Implications for communication research on clinical teams are discussed. Conclusion:Implications are provided for the variables along with a discussion of the findings from the data. Practical implications for healthcare teams are also discussed for consideration of team member behaviors in the future. Suggestions for future research are identified.]]></abstract>

				<title>Reducing Medication Errors through Multi-Disciplinary Collaboration: A Quality Improvement initiative</title>

				<abstract><![CDATA[Introduction:Prescribing and medication administration errors are common themes in Pediatrics. There is growing international evidence that the regular occurrence of such errors carries a high potential for unintended harm to patients. Within our Trust, a high percentage of reported pediatric incidents relate to medication errors. The most-commonly reported themes were incorrect dosing and omission of regular medication. The aim of our project was to reduce medication errors by at least 10%. Materials and Methods: To achieve our aim, we devised a structured educational program was devised by a tripartite alliance (Nursing, Medicine, Pharmacology) and rolled out to nursing staff and medical trainees. An initial prospective audit was undertaken, followed by two PDSA (Plan-Do-Study-Act) cycles. Results: Following the intervention, the percentage of medication errors decreased from 89.3% to 12.1%, with a comparative 51.3% decrease in significant errors and a complete elimination of serious/potentially lethal errors. Conclusion:In view of our results, we hope that tripartite alliances may be used as a model for inter-professional collaboration across healthcare systems.]]></abstract>

				<title>Does a Dedicated Frailty Service Improve Outcomes for Emergency Laparotomy in Elderly Patients?</title>

				<abstract><![CDATA[Introduction:With improvements in medical care, more and more patients are surviving into old age. Elderly patients presenting with abdominal surgical emergencies constitute the majority of an emergency surgeon’s workload. Recently, there has been a lot of interest in improving surgical outcomes for frail and elderly patients needing emergency laparotomies. We carried out a prospective audit to investigate the impact of having a dedicated frailty service on the post-operative outcomes of such frail elderly patients. Materials and Methods: Our audit included all elderly patients (defined as &gt; 65 years of age at the time of surgery) undergoing emergency laparotomy for a period of six months from January to June 2021 in our 700-bed acute care hospital in the UK. Post-operative outcomes were 30 and 90-day mortality, length of stay in hospital, and proportion of patients discharged to their own home after surgery. Results: Our data demonstrates that the availability of such a dedicated frailty service does significantly decrease the proportion of patients discharged to their own homes and the rate of readmissions after emergency laparotomies. Length of stay, 30 and 90 day mortalities decreased to a non-significant level. Conclusion:Therefore, establishing a dedicated frailty service for both preoperative optimization as well as improving post-operative care is an important step to improving outcomes for this group of high-risk surgical patients.]]></abstract>

				<abstract><![CDATA[Introduction:Providing health services in medical centers is associated with risks and medical errors for recipients. In many cases, despite medical experts’ ethical and professional commitment to disclose errors, the error report among treatment staff is less than the actual rate. Therefore,, the present study was conducted to investigate using a new electronic error-reporting registration system method in Shohadaye Haftom Tir Hospital in 2018-2019. Materials and Methods: This was a descriptive cross-sectional study. Errors reported in 2018 and 2019 were investigated using the paper-based method and electronic error-reporting system, respectively. Data were analyzed using SPSS software (version 22) after classifying errors.  Results: In the electronic method, 194 errors were reported, of which medication errors accounted for 33% and medical errors for 22% of the total errors. Hospital congestion was one of the major causes of the error. Nurses were more likely to make medical errors than other groups. In the paper-based reporting error method, out of 44 cases, about 37% were technical-related errors. The most important cause of the error was a lack of proper communication with the patient (30%). Findings suggested that the error percentage significantly increases in both methods due to the causes, specific sectors, and job categories. Conclusion:Findings have shown the efficiency of the electronic system in detecting errors. Managers` attention and actions toward using the electronic system can greatly affect the health and treatment of patients.]]></abstract>
